Any additional services or fees will be discussed with you in advance whenever possible.
Cancellation and No-Show Policy
Please provide at least 24 hours’ notice when canceling or rescheduling an appointment. Late cancellations and missed appointments may result in a $50 fee. Please review the Financial Policy for complete details.
Payment
Payment is due at the time services are provided. A valid payment method may be required to remain securely on file.
Good Faith Estimates
If you do not have insurance or choose not to use insurance for your care, you have the right to receive a Good Faith Estimate explaining the expected cost of your services.
You may request a Good Faith Estimate before scheduling, or one will be provided when required for scheduled services. A Good Faith Estimate is not a bill and may not include services that could not reasonably be anticipated before treatment

Insurance
Sycamore Mental Health & Wellness is currently a self-pay practice and does not bill insurance. Payment is due in full at the time of service.
If your plan includes out-of-network mental health benefits, you may be eligible for partial reimbursement. Upon request, we can provide a superbill for you to submit directly to your insurance company. Reimbursement is determined by your insurance plan and is not guaranteed.
Planning to Use Out-of-Network Benefits?
When contacting your insurance provider, consider asking:
Does my plan include out-of-network benefits for outpatient psychiatric care?
Do I have an out-of-network deductible?
What percentage of the cost will my plan reimburse?
How do I submit a superbill for reimbursement?
Are telehealth visits with an out-of-network psychiatric provider eligible for reimbursement?
